Filled out a NEW registration form?
Filled out a NEW medication form? Remember that this form is needed for prescriptions and over the counter meds. Your doctor has to sign this form.
Provided a copy of your insurance card?
Provided a copy of immunizations?
Read the "Rules & What to Bring" document? (located under forms)
If you take medication (Rx or OTC), a parent MUST accompany you to registration to sign the appropriate paperwork!
REGISTRATION SIGN IN / SIGN OUT
On Wednesday - arrive no earlier than 2pm
(partial stay is possible, just see the Nurse if you arrive at a later date)
Bring a parent/guardian
Bring of all your required paperwork (see above)
On the last Sunday - You MUST be moved out of the dorm and signed out by a parent/guardian no later than 12 noon
Rachel has been faithfully attending Mooers Camp Meeting for over 20 years and has been working as a camp nurse for about 4 years. She has been an RN for 6 years, working with and ministering to, patients battling cancer and those who are facing end of life.
"I have been following the Lord's leading for over 25 years. I have a wonderful father and mother (pastor and wife team for the Mooers Wesleyan Church), two amazing sisters, two great brothers-in-law and two beautiful nieces whom I get to spoil. I LOVE working with the Mooers Camp Youth. They hold a special place in my heart, and I am excited to see what the Lord has in store for us this year."
